Endodontics

Endodontics is a relatively young but increasingly key discipline for dentistry that is developing at a breakneck pace. As its name suggests, it deals with treatment inside (endo) the tooth (odont). And the ultimate goal of this treatment is to save the diseased tooth from extraction.

This is necessary in case of infection of tissues and nerves in the core of the tooth, which can form pulpitis (complicated and untreated caries), subsequently granuloma and even abscess. To prevent this severe and highly painful prospect, modern endodontics now has a wide range of effective means at its disposal. But the most important of them is a timely examination by a specialist as soon as you feel pain in the tooth or increased sensitivity.

What endodontics does

Simply put, endodontics deals with root canal treatment. For the general public, this procedure is known as “nerve culling” or “nerve extraction”. And it’s not new. New are the means used by modern science to preserve the tooth and prevent its replacement with bridges, implants and crowns.

For this purpose, the procedure involves careful cleaning of the root canals from the infected tissue, thorough disinfection and subsequent filling with a special material. All of this must be done under a microscope because extreme precision is required.

Treatment is carried out in several stages, and its duration depends on the anatomy of the masticatory apparatus, its location and the number of roots:

Placement of anaesthetic

Before removing a nerve, it is absolutely necessary to place a local anesthetic. The reason is that exposed and inflamed pulp is very painful even at the slightest touch. The endodontist will decide whether to take this approach. The alternative is to insert a drug whose action over several days will cause the nerve to die.

Once the nerve of the tooth has lost sensitivity, we proceed to open and expose the pulp chamber under a microscope.

Extraction of the nerve

Next comes the biggest challenge for the endodontist – removing the nerve from the root canals and the tooth chamber. When it is completed, the channel is cleaned and prepared for filling by widening and shaping.

The final stage is the filling of the root canal with a special paste, which then hardens, seals the inside of the tooth and thus protects it from the entry of microorganisms and the resurgence of infection.

Radiography

We will take an x-ray after treatment is complete. It will help us to get a sense of the results of – we promise – a job well done, as well as to follow developments in the case going forward. However, experience has shown that at Medical Dent Dental Clinic, once the endodontic treatment is completed, the case is over once and for all.

It is essential that endodontic treatment is carried out in a clean, sterile and protected environment. To this end, the number one task is to isolate the tooth being treated prior to nerve extraction. This is done with a special latex canvas we call coferdam. It covers the tooth tightly and prevents the penetration of saliva and bacteria from neighbouring teeth and from the oral cavity during the entire treatment process.

There are many cases of patients coming to us after root canal treatment which unfortunately has not been successful. This is sometimes due to overly complex tooth anatomy, insufficient precision on the part of the previous treating endodontist when extracting the nerve, or broken root canal instruments. Then we will have to re-treat the already treated root canals.

We are also prepared to meet the challenging cases where we have a self-dead nerve. This most often occurs with a strong blow, which abruptly breaks the connection of the dental pulp with the remaining tissues. Or due to a “silent” infection, which initially does not give painful symptomatology, but kills the nerve. In these cases also the intervention of the endodontist and the extraction of the nerve from the tooth are absolutely necessary to prevent the spread of inflammation.
